Das Prostatakarzinom ist der häufigste und wichtigste maligne Tumor bei Männern. Bei den meisten operablen Patienten besteht die Therapie der Wahl in einer chirurgischen Entfernung der Prostata und gegebenenfalls einer Nachbestrahlung. Jährlich sterben circa 13.000 Männer in Deutschland an einem Prostatakarzinom. Mit der Verfügbarkeit von neuen Medikamenten, welche auch eine effektive Chemotherapie des Prostatakarzinoms zulassen, ergibt sich nun neu auch die Frage einer adjuvanten medikamentösen Therapie bei Hochrisiko-Patienten und damit gleichzeitig auch die Frage, wie Hochrisiko-Patienten zu definieren sind. In diesem Themenkreis spielt die Lymphadenektomie beim Prostatakarzinom eine zentrale Rolle. Auf der einen Seite ist der Nachweis von Lymphknoten-Metastasen ein entscheidend wichtiger Prognosefaktor und deswegen für die Definition „Hochrisiko-Patienten“ wichtig. Auf der anderen Seite ist die ausgedehnte Lymphadenektomie wahrscheinlich auch eine probate „adjuvante“ Behandlung. Verschiedene Studien deuten tatsächlich darauf hin, dass eine ausgedehnte Lymphadenektomie beim fortgeschrittenen Prostatakarzinom eine kurative Rolle spielen kann, zumindest wenn die Metastasierung auf die operativ entfernten Lymphknoten beschränkt war. Leider ist die Lymphadenektomie aber auch mit einer signifikanten Morbidität verbunden. Symptomatische Lymphozelen kommen in circa 2,3-10,3% nach Lymphadenektomie vor. Sie sind umso häufiger, je gründlicher die Lymphadenektomie erfolgte und bedürfen oft einer langwierigen, auch operativen Behandlung. Aus diesem Grund muss die Indikationsstellung zur Lymphadenektomie mit Augenmaß erfolgen. Die Bedeutung der prognostischen Information und die mögliche therapeutische Bedeutung müssen gegenüber dem Risiko einer Lymphozele abgewogen werden. Ziel dieser Doktorarbeit war es 1. zu klären, ob eine bessere Gleason-Gradierung des Primärtumors das Risiko einer Lymphknoten-Metastasierung besser abschätzen lässt als die traditionelle Gleason-Gradierung und 2. zu klären, ob eine bessere Quantifizierung der Metastasierungsmenge und der Nachweis einer extranodalen Ausbreitung der Lymphknoten-Metastasen, eine bessere Einordnung der Gefährlichkeit von Lymphknoten-Metastasen erlaubt. Zu diesem Zweck wurden mehr als 10.000 Patienten der Martini-Klinik untersucht. Es handelt sich um 12.790 Patienten, welche wegen Prostatakarzinom prostatektomiert wurden und bei denen auch eine Lymphadenektomie durchgeführt worden war. Es handelt sich um 11.280 N0- und 1.510 N1-Patienten. Von allen Patienten wurden alle Lymphknoten-Metastasen nachbeurteilt und ausgemessen. Bestimmt wurde die gesamte Fläche aller Lymphknoten-Metastasen und ob eine extranodale Ausbreitung vorlag oder nicht. Die Metastasierungsparameter wurden zusätzlich mit dem quantitativen Gleason-Grad verglichen. Der quantitative Gleason-Grad stellt eine Verbesserung der traditionellen Gleason-Gradierung dar, wobei Subgruppen gebildet werden mit unterschiedlichen Gleason 4 Anteilen. Die Daten machen deutlich, dass das Metastasierungsrisiko mehr oder weniger linear mit dem Anteil von ungünstigen Gleason-Graden zunimmt. Der quantitative Gleason-Grad erweist sich somit als wesentlich besseres Mittel zur Metastasen-Prädiktion als der traditionelle Gleason-Grad. Innerhalb der N1-Patienten erweist sich die Gesamtfläche an Metastasen als relevanter Prognosefaktor. Die Gesamtfläche von Metastasen korreliert eng mit dem quantitativen Gleason-Grad und dem pT-Stadium und dem Risiko eines PSA-Rezidivs. Im Gegensatz dazu kommt dem bei 731 von 1.510 N1-Patienten nachgewiesenen extranodalen Tumorwachstum keine klinisch relevante prognostische Bedeutung zu. Zusammengefasst zeigen die Ergebnisse dieser Untersuchung, dass der quantitative Gleason-Grad ein potenter Prädiktor für das Vorliegen von Lymphknoten-Metastasen ist. Die Gesamtfläche der nachgewiesenen Lymphknoten-Metastasen ist bei N1-Patienten hoch signifikant mit der Prognose assoziiert. Die Metastasengesamtfläche dürfte somit ein wichtiger Parameter bei der Entscheidung sein, ob eine adjuvante chemotherapeutische Maßnahme bei N1-Patienten sinnvoll ist oder nicht.

Prostate cancer ist the most common form of malignant cancer among men. For the majority of cases where surgery is an option, the most commonly chosen treatment consists of a surgical removal of the prostate, and if necessary a post-surgery irradiation therapy. Overall the annual mortality from prostate cancer amounts to approximately 13.000 men across germany. The increased availability of new drugs, which allows an effective chemotherapy treatment of prostate cancer, puts a further focus on the issues of an adjuvant medicamenteous treatment of high-risk patients. Hence, this also puts further attention on the issue of the definition of high-risk patients. Lymphadenectomy plays a central role in “high risk“ prostate cancer patients. On the one hand, detection of lymph node metastases is a crucial prognostic factor and therefore important for the definition of „high-risk“ patients. On the other hand, the extended lymphadenectomy might also be suitable „adjuvant“ treatment. Several studies provide evidence for a cruative role of extended lymphadenectomy for advanced prostate cancer, in particular for cases where metastases were limited to the surgically removed lymph nodes. Unfortunately lymphadenectomy is also associated with an increased morbidity. In approximately 2.3-10.3 % symptomatic lymphoceles occure following lymphadenectomy. These occure more frequently in case of extensive lymphadenectomy, and often require prolonged and even operative treatment. For this reason, the indication for lymphadenectomy must be evaluated with care. The importance of prognostic information, and the potential therapeutic importance must be weighted against the risk of lymphocele. The aim of this study was hence to clarify 1. whether a better Gleason grading of the primary tumor allows for a more accurate estimation the risk of lymph node metastases than the traditional Gleason grading and 2. if more percise quantifications of metastatic mass and evidence of extranodal extension of lymph node metastases allow for a better classifications of the dangerousness of lymph node metastases. For this work, more than 10.000 patients of the Martini-Klinik were examined. There were 12.790 patients who underwent a radical prostatectomy due to prostate cancer, and where also a lymphadenectomy was performed. The study includes 11.280 N0 and 1.510 N1 patients. All lymph node metastases of all patients were re-evaluated and measured. The total area of lymph node metastases was determined and it was testet whether an extranodal extension was present or not. The metastases parameters were additionally compared to the quantitative Gleason grade. The quantitative Gleason grade is an enhancement of the traditional Gleason grading and is formed in subgroups with different Gleason 4 pattern proportions. The data shows that the risk of metastases increases approximately linearly with the proportion of unfavorable Gleason grades. The quantitative Gleason grade proves to be a more accurate predictor of metastases than the traditional Gleason grade. Within the group of N1 patients, the total area of metastases was determined to be a relevant prognostic factor. The total area of metastases was observed to closley correlate with the quantitative Gleason grade, the pT-stage, and the risk of a biochemical recurrence. In contrast, the extranodal extension of lymph node metastases detected in 731 out of 1.510 N1 patients has no clinical relevant prognostic significance.

In summary, the results of this study indicate that the quantitative Gleason grade is a potent predictor for the presence of lymph node metastases. The total area of detected lymph node metastases is significantly associated with the outcome across N1 patients. The total area of metastases should therefore be an important parameter when deciding whether an adjuvant chemotherapeutic treatment in N1 patients is usefull or not.
