Zusammenfassung: 
Ziel: Ziel dieser Studie war es, den Erythropoietin-Serumspiegel (EPO) bei Patienten mit akutem Myokardinfarkt und instabiler Angina pectoris zu bestimmen.
Methoden: Es wurden 17 Patienten mit einem akuten Myokardinfarkt (AMI-Gruppe), 11 Patienten mit instabiler Angina pectoris IIIB (AP-Gruppe), 18 gesunde Probanden (Kontrolle-1) und 10 Patienten mit instabiler Angina pectoris und Ausschluss einer koronaren Herzerkrankung (Kontrolle-2) eingeschlossen. Es wurde bei allen Patienten (ausser bei den gesunden Probanden) eine Koronarangiographie durchgeführt. Bei allen Patienten in der AMI-Gruppe wurde anschliessend eine Koronarangioplastie und Stentimplantation durchgeführt. Der EPO-Serumspiegel wurde bei den Patienten der AMI- und AP-Gruppen an 3 prädefinierten Zeitpunkten bestimmt: Probe-1= 1-2, Probe-2= 3-5 und Probe-3= 5-9 Tage nach dem akuten Ereignis. In den Kontrollgruppen erfolgte nur eine EPO-Bestimmung. Die Bestimmung des EPO-Spiegels erfolgte mittels ELISA. Es erfolgte auch der Vergleich zwischen dem EPO-Serumspiegel und den folgenden Parametern: Kreatinkinase, Hämoglobin, Diabetes mellitus, familiäre Vorbelastung, Hyperlipidämie, arterielle Hypertonie und Nikotinkonsum.
Ergebnisse: Der EPO-Serumspiegel ist sowohl in der AMI-Gruppe als auch in der AP-Gruppe signifikant erhöht gegenüber den Kontrollgruppen (AMI vs Kontrolle-1, p= 0,0001; AMI vs Kontrolle-2, p= 0,0002; AP vs Kontrolle-1, p= 0,0074; AP vs Kontrolle-2, p= 0,014). Der EPO-Spiegel ist in der AMI-Gruppe nicht signifikant erhöht (p= 0,150) gegenüber der AP-Gruppe. Der EPO-Spiegel in den AMI- und AP-Gruppen weist ein Maximum in den ersten 2 Tagen nach dem akuten Ereignis auf und ist unabhängig von: Kreatinkinase, Hämoglobin und den klassischen kardiovaskulären Risikofaktoren.
Conclusio: Patienten mit einem akuten Myokardinfarkt oder instabiler Angina pectoris weisen einen erhöhten EPO-Serumspiegel auf. Dieser Spiegel erreicht ein Maximum in den ersten 2 Tagen nach dem akuten klinischen Ereignis und ist unabhängig von den Kreatinkinase- und Hämoglobinwerten im Serum.

Objective: The objective of this study was to measure the serum-level of erythropoietin in patients with acute myocardial infaction and unstable angina pectoris.
Methods: There were 17 patients with acute myocardial infarction (AMI-group), 11 patients with unstable angina pectoris (AP-group), 18 healthy subjects (Control-1) and 10 patients with unstable angina pectoris and no angiographic evidence of coronary heart disease (Control-2) included in the study. All patients (except the healthy subjects) have undergone a coronary angiography. All patients in the AMI-group were treated with angioplasty and stent-implantation. The serum-level of erythropoietin in both the AMI- and the AP-groups was measured at three predetermined time-points: probe-1 = day 1-2, probe-2 = day 3-5 and probe-3 = day 5-9 after the onset of clinical symptoms. In both control-groups there was only one measurement of erythropoietin in serum. The EPO-level in serum was determined using ELISA-test. A search for a correlation of the measured EPO-levels to parameters as hemoglobin, creatinkinase, diabetes mellitus, familiary predisposition, hyperlipidemia, arterial hypertension and cigarette-smoking was also performed.
Results: The serum-level of erythropoietin in both the AMI- and the AP-groups was significantly higher than in the control-groups (AMI vs Control-1, p= 0,0001; AMI vs Control-2, p= 0,0002; AP vs Control-1, p= 0,0074; AP vs Control-2, p= 0,014). The EPO-level in the AMI-group was not significantly higher than in the AP-group (p= 0,150). In both the AMI- and the AP-group the EPO-level shows a maximum in the first 2 days after the clinical event and is independent of the levels of hemoglobin, creatinkinase and the presence of the examined cardiovascular risk factors.
Conclusion: The patients with an acute myocardial infarction as well as the patients with unstable angina pectoris in this study have a significantly elevated serum-level of EPO. This level shows a maximum in the first 2 days after the clinical event and is independent of the levels of hemoglobin and creatinkinase.
