Zusammenfassung: 
Es wurde eine retrospektive Analyse der Behandlungs-ergebnisse von 182 Patientinnen mit Zervixkarzinom, die an der Universitätsklinik Hamburg-Eppendorf zwischen 1995-2005 operiert wurden, durchgeführt.Untersucht wurden Prognosefaktoren sowie Prädiktionswert bildgebender Verfahren und klinischer Untersuchung.
Die univariate Analyse der Prognosefaktoren ergab einen signifikanten Einfluss von Alter (p=0,003), Tumorstadium (p=0,0000005 und p=0,000003), Nodalstatus (p=0,00000002 und p=0,00000001), Parametrieninvasion (p=0,0000000001 und p=0,0000000007), Lymphangiosis carcinomatosa (p=0,007 und p=0,027), Resektionsstatus (p=0,00006 und p=0,0002) und erhöhte prätherapeutische SCC-Ag Werte (p=0,007 und p=0,02) auf die rezidivfreie- und Gesamtüberlebenszeit.In der multivariaten Analyse stellte sich der Nodalstatus als einziger signifikanter und unabhängiger Prognosefaktor dar (p=0,03 und p=0,04).
Weiterhin wurde der Stellenwert der klinischen Untersuchung, der Computertomographie (CT) und der Kernspinntomographie (MRT) zur prätherapeutischen
Festlegung des Tumorstadiums (FIGO)und des Nodalstatus
bei Patientinnen mit operablem Zervixkarzinom untersucht. Die präoperative Stadieneinteilung mittels klinischer Untersuchung war der Einteilung mit bildgebenden Verfahren (MRT und CT) in frühen Stadien weit überlegen. MRT und CT wiesen eine hohe Spezifität, bei jedoch nur geringer Sensitivität im Nachweis von Lymphknotenmetastasen auf. Beide bildgebenden Verfahren geben insbesondere bei frühen Tumorstadien keine zusätzliche diagnostische
Sicherheit, und sollten deshalb keinen Einfluss auf die Therapieentscheidung haben.

The objective of this study was to identify the prognostic factors of surgically treated cervical carcinomas. Furthermore the diagnostic accuracy of clinical evaluation, MR imaging and computed tomography were compared. The records of 182 patients with invasive cervical carcinoma who underwent surgical treatment at the University Hospital Hamburg-Eppendorf from 1995 to 2005 were retrospectively reviewed. Clinical and pathologic variables including age, clinical stage (TNM), lymphovascular space involvement, cell type, tumor grade, lymph node metastases, parametrial invasion, resection status and the pretreatment serum squamous cell carcinoma antigen (SCC-Ag) levels. The prognostic significance of these factors were evaluated using univariate and multivariate methods. Results: Univariate analysis revealed that age (p=0,003), clinical stage (p=0,0000005 and p=0,000003), lymph node metastases (p=0,00000002 and p=0,00000001), parametrial invasion (p=0,0000000001 and p=0,0000000007), Lymphangiosis carcinomatosa (p=0,007 and p=0,027), surgical margin involvement (p=0,00006 and p=0,0002) and pretreatment elevated SCC-AG levels (p=0,007 and p=0,02) were significantly associated with disease-free survival and overall survival. Of these, the Cox proportional-hazard model revealed that only the lymph nodes metastases was an independent prognostic factor (p=0,03 and p=0,04). Conclusions: Lymph node metastases is the only independent prognostic factor in cervical carcinoma who underwent radical surgical treatment.
Furthermore this study was designed to compare the diagnostic accuracy of clinical evaluation, MR imaging and computed tomography in patients with cervical cancer. The detection of lymph node metastases in tomography and MR imaging was additionally analyzed. A retrospective analysis was carried out of 190 patients with cervical cancer who underwent surgical treatment at the University Hospital Hamburg-Eppendorf from 1 Jan 1995 to 31 Dec 2005. The results of 157 clinical evaluations, 41 MR imagings and 35 computed tomographies were compared to the postoperative histopathological findings. Results: In 76 % the clinical staging corresponded to the histopathological findings. Staging with MR and computed tomography was identical with pathology in 46 and 34 %, respectively. The stage of disease was overestimated in 14 % of the clinical evaluations, in 34 % of MR imagings and in 37 % of the computed tomographies. Underestimation occurred in 10 % of the clinical evaluations, in 20 % of the MR imagings and in 29 % of the computed tomographies. The sensitivity and specificity for the detection of lymph node metastases were 33 and 88 % for computed tomography and 30 and 90 % for MR imaging. Conclusions: In patients with cervical cancer, clinical evaluation is superior to both tomography and MR imaging for the determination of the FIGO stage.
