In der vorliegenden Arbeit wurde an einem Kollektiv von 50 Patienten mit duktalem Pankreasadenokarzinom die Häufigkeit der HER2-Genamplifikation und der Ploidiegrad mittels Chromosom 17 ermittelt. Eine HER2-Genamplifiaktion war mit 24% ein häufiges Ereignis, führte aber nur bei high-level Amplifikation zu einer p185HER-Überexpression (4% der Fälle). Weder die HER2-Amplifikation noch die p185HER-Überexpression zeigten eine prognostische Signifikanz. Anhand der Ploidie von Chromosom 17 konnten die Tumore in eine disome (n=19), eine trisome (n=19) und eine hypertetrasome Gruppe (n=12) aufgeteilt werden. Die Aneuploidie für Chromosom 17 stellte sich dabei als signifikanter unabhängiger prognostischer Faktor für das Gesamtüberleben heraus.
Anschließend wurden bei einem Kollektiv von 52 Patienten mit duktalem Pankreasadenokarzinom mittels Lasermikrodissektion Tumorzellen isoliert, nach der SCOMP Methode global amplifiziert und anschließend mittels komparativer genomischer Hybridisierung auf Metaphasen analysiert. Die häufigsten Genvermehrungen waren bei on 2q, 8q, 4q, 3q, 1q und 13q, die häufigsten Verluste bei on 17p, 18q, 19p, 6q, 8p, 9p, 22q und 1p lokalisiert. Generell waren diese Veränderungen mit den Ergebnissen der vorangegangenen Studien kompatibel. Dabei zeigten sich die Deletionen auf dem q-Arm von Chromosom 4 sowie das Vorhandensein von Lymphknotenmetastasen als unabhängige Prognosefaktoren. Die Deletionen auf 4q und Lymphknotenmetastasen stellten auch in der multivariaten Datenanalyse einen unabhängigen prognostischen Faktor für das tumorfreie Überleben dar.
Eine Möglichkeit der weiteren Investigation wäre die Untersuchung mit hochauflösenden Methoden wie der Array-CGH, um die zugrundeliegenden Gene und Genregionen zu identifizieren, die ursächlich an der Entstehung der chromosomalen Instabilität beteiligt sind.

In the first part the aim was to determine the frequency and the potential clinical use of HER2 (17q21) gene amplification and chromosome 17 aneuploidy in pancreatic ductal adenocarcinoma (PDAC). Serial tissue sections of 50 resected PDACs were analyzed with chromogenic in situ hybridization using locus-specific HER2 probes and centromeric probes for chromosome 17. Centromeric probes for chromosome 7 and 8 were hybridized to confirm ploidy levels. Expression of HER2 protein was assessed by immunohistochemistry. Correlations of experimental findings with clinical and follow-up data were tested. The HER2 gene locus was frequently (24%) amplified in PDAC and the rate of overexpression (2+ and 3+) was 10%, but no prognostic significance was found. Copy number analysis of chromosomes 7, 8, and 17 revealed disomic (40%), trisomic (36%), and hypertetrasomic (24%) tumors. Compared with patients with disomic tumors, patients with hypertetrasomic tumors exhibited a significantly decreased relapse-free and overall survival (5.0 v 13.0 months, P = .0144 and 7.0 v 20.0 months, P = .0099, respectively). Multivariate analysis confirmed the independent prognostic significance of hypertetrasomy. Tumor ploidy levels correlate with prognosis of PDAC patients, indicating characteristic biologic properties of PDAC with high chromosomal instability. In contrast, no prognostic influence on patient outcome was found for the amplification of the HER2 oncogene or p185(HER2) overexpression. Therefore, evaluation of ploidy levels offers new opportunities for patient stratification in clinical trials and enables novel approaches to study the well-known aggressiveness of PDAC.

In the second part the prognostic impact of genomic alterations in operable localized pancreatic ductal adenocarcinoma (PDAC) was tested. Fifty-two formalin-fixed and paraffin-embedded primary PDAC were laser micro-dissected and were investigated by comparative genomic hybridization after whole genome amplification using an adapter-linker PCR. Chromosomal gains and losses were correlated to clinico-pathological parameters and clinical follow-up data. The most frequent aberration was loss on chromosome 17p (65%) while the most frequent gains were detected at 2q (41%) and 8q (41%), respectively. The concomitant occurrence of losses at 9p and 17p was found to be statistically significant. Higher rates of chromosomal losses were associated with a more advanced primary tumor stage and losses at 9p and 18q were significantly associated with presence of lymphatic metastasis (chi-square: p = 0.03, p = 0.05, respectively). Deletions on chromosome 4 were of prognostic significance for overall survival and tumor recurrence (Cox-multivariate analysis: p = 0.026 and p = 0.021, respectively). In conclusion our data suggest the common alterations at chromosome 8q, 9p, 17p and 18q as well as the prognostic relevant deletions on chromosome 4q as relevant for PDAC progression. The comprehensive data from 52 PDAC should provide a basis for future studies with a higher resolution to discover the relevant genes located within the chromosomal aberrations identified.
