Das Neuroblastom ist der häufigste extrakraniale solide Tumor des Kindesalters und verantwortlich für 11 % der durch pädiatrische Tumorerkrankungen verursachten Todesfälle. Es weist eine ausgeprägte Heterogenität bezüglich des klinischen Verlaufs auf. Anhand von Risikofaktoren wie INSS-Stadium, Alter bei Diagnose, MYCN- und 1p-Status werden Neuroblastome verschiedenen Risikogruppen zugeordnet. Die pY-abhängige Signaltransduktion spielt eine erhebliche Rolle in der Pathogenese des Neuroblastoms, z.B. werden in etwa 10 % der Neuroblastome aktivierende Mutationen der RTK ALK beobachtet.
In der vorliegenden Arbeit wurde eine umfassende Analyse der pY-abhängigen Signaltransduktion im Neuroblastom mittels SH2-Profiling durchgeführt. Dabei wurden in Far-Western Blot-Analysen mit Hilfe von 35 bzw. 16 SH2-Domänen globale pY-Signalprofile von neun Neuroblastom-Zelllinien bzw. 21 primären Neuroblastomen verschiedener Risikogruppen erhoben und auf Zusammenhänge mit klinischen Parametern untersucht. Parallel dazu wurden mittels Western Blot-Analysen und ELISA 26 potenziell relevante Proteinspezies, darunter 17 RTKs, in den Primärtumoren bestimmt und auf ihre Verteilung auf die Risikogruppen sowie auf Assoziationen mit den SH2-Profilen analysiert. Abschließend wurden aus den erhobenen SH2-Profilen in Niedrig- und Hochrisiko-Neuroblastomen differenziell phosphorylierte pY-Proteine ermittelt.
Clusteranalysen der SH2-Profile der Neuroblastom-Zelllinien zeigten eine bezüglich klinischer Parameter interessante Gruppeneinteilung der Zelllinien und ermöglichten eine adäquate Auswahl an SH2-Domänen für das SH2-Profiling primärer Neuroblastome. Die Bestimmung verschiedener Proteinspezies in den Primärtumoren bestätigte eine risikoabhängige Expression von ALK, N-Myc, Shc3, TrkA und Fyn, für die verbleibenden Proteinspezies hingegen, darunter auch TrkB und TrkC, wurde kein Zusammenhang mit dem Risiko beobachtet. Mehr als die Hälfte der untersuchten RTKs war in über 75 % aller Tumoren, EGFR, IGF1R, KIT und PDGFRβ sogar in sämtlichen Neuroblastomen exprimiert. Das SH2-Profiling primärer Neuroblastome zeigte ähnliche, globale Profile pY-abhängiger Signaltransduktion ohne offensichtliche Zusammenhänge zu den bestimmten Proteinspezies. Die Clusteranalyse der SH2-Profile ordnete die Tumoren drei Clustern zu, die signifikant mit dem Risiko, dem INSS-Stadium sowie dem Alter bei Diagnose assoziiert waren. Schließlich konnten aus den SH2-Profilen sieben pY-Proteine ermittelt werden, die in Niedrig- und Hochrisiko-Neuroblastomen differenziell phosphoryliert waren.
Die erhaltenen Ergebnisse zeigen, dass globale Profile pY-abhängiger Signaltransduktion im Neuroblastom tumorrelevante Informationen in Form einer komplexen Kombinatorik zahlreicher pY-Proteine enthalten und möglicherweise bei der Risikostratifizierung des Neuroblastoms behilflich sein könnten. Die ubiquitäre Expression mehrerer RTKs im Neuroblastom empfiehlt eine genauere Untersuchung der Rolle dieser in der Pathogenese des Neuroblastoms sowie der möglichen Implikationen für eine zielgerichtete Therapie. Die aus den SH2-Profilen ermittelten, differenziell phosphorylierten pY-Proteine stellen potenzielle Kandidaten für eine zukünftige Identifizierung und funktionelle Validierung als mögliche neue therapeutische Ziele dar.

Neuroblastoma is the most frequent extracranial solid tumor of childhood and accounts for 11 % of pediatric cancer deaths. It shows a remarkable heterogeneity in clinical behavior. Neuroblastomas are classified into different risk groups based on risk factors like INSS stage, age at diagnosis, MYCN status and 1p status. pY-dependent signal transduction seems to be crucial for neuroblastoma pathogenesis, activating mutations of the RTK ALK, for example, are found in 10 % of neuroblastomas.
This thesis presents a comprehensive analysis of pY-dependent signal transduction in neuroblastoma. Global profiles of pY signaling of nine neuroblastoma cell lines as well as 21 primary tumors of different risk groups were obtained by far-western blot analysis using 35 and 16 SH2 domains, respectively, and analyzed for associations with clinical parameters. In parallel, expression levels of 26 potentially relevant protein species, 17 of which were RTKs, were determined and examined for their distribution over the risk groups and for associations with the SH2 profiles. Finally, SH2 profiles were searched for differentially phosphorylated proteins among low and high risk neuroblastomas.
Cluster analyses of the SH2 profiles of the neuroblastoma cell lines showed interesting results with regard to clinical parameters and provided an adequate selection of SH2 domain probes for the SH2 profiling of primary neuroblastomas. Determination of the expression levels of several protein species in the primary tumors confirmed a risk-associated expression of ALK, N-Myc, Shc3, TrkA, and Fyn, whereas the remaining protein species including TrkB and TrkC were not associated with risk. More than half of the analyzed RTKs were expressed in over 75 % of neuroblastomas, with EGFR, IGF1R, KIT, and PDGFRβ being expressed in all of the tumors. SH2 profiling revealed similar global profiles of pY-dependent signal transduction in primary neuroblastomas with no apparent associations to the analyzed protein species. Cluster analysis of the SH2 profiles assigned the tumors to three clusters that were significantly associated with risk, INSS stage and age at diagnosis. Finally, seven pY proteins that were differentially phosphorylated among low and high risk neuroblastomas could be determined.
The presented results demonstrate that global profiles of pY-dependent signal transduction in neuroblastoma contain tumor-relevant information in the form of complex combinations of numerous pY proteins and might aid neuroblastoma risk stratification. The finding of a ubiquitous expression of several RTKs in primary neuroblastomas recommends a closer investigation of the role of these RTKs in the pathogenesis of neuroblastoma and the possible implications for therapy. The differentially phosphorylated proteins obtained from the SH2 profiles represent potential candidate proteins for future identification and functional validation as possible new therapeutic targets.
