Zusammenfassung: 
Bezüglich der Arzneimitteltherapie älterer Patienten besteht ein Ungleichgewicht: Einerseits ist die in Deutschland bedingt durch den demographischen Wandel stetig wachsende Gruppe der Patienten ab 65 Jahre die bedeutendste Zielgruppe der Pharmakotherapie. Andererseits werden ausgerechnet diese älteren Patienten häufig aus Studien ausgeschlossen. Es ist wichtig, die Arzneimitteltherapie an die speziellen Bedürfnisse der älteren Patienten anzupassen. Eine Möglichkeit ist den Einsatz von potentiell inadäquater Medikation (PIM) zu vermeiden. PIM für ältere Patienten sind Arzneimittel, bei deren Anwendung ein hohes Risiko für unerwünschte Arzneimittelwirkungen besteht und für die es gleichzeitig eine sicherere Alternative gibt. Um PIM zu identifizieren sind eine Reihe von Kriterien-Katalogen entwickelt worden. In der vorliegenden Untersuchung wurden FORTA (fit for the aged), PRISCUS (lateinisch: priscus, zu Deutsch: altehrwürdig) und STOPP (Screening Tool of Older Person’s potentially inappropriate Prescriptions) zur Identifikation von PIM genutzt. Ziel der Studie war herauszufinden, welche PIM wie häufig angewendet werden und wie während des stationären Aufenthaltes am Universitätsklinikum Hamburg-Eppendorf (UKE) mit den PIM verfahren wird. Außerdem wurden ein möglicher Zusammenhang zwischen PIM und Hauptdiagnose sowie schwerwiegenden Auswirkungen während des stationären Aufenthaltes untersucht. Im Rahmen einer retrospektiven Erhebung wurde in vier Punktprävalenz-analysen die Medikation von 200 Patienten bei Aufnahme, während des stationären Aufenthaltes und bei Entlassung anhand von FORTA, PRISCUS und STOPP unter Einbeziehung der Diagnosen und klinischer Parameter analysiert. In die Untersuchung eingeschlossen wurden nicht-elektive Patienten mit einem Alter von ≥ 65 Jahren, die mindestens fünf Arzneistoffe bei Aufnahme über die zentrale Notaufnahme einnahmen. Das Durchschnittsalter der Patienten war 78,8 Jahre, 60,5 % waren weiblich. 176 der 200 Patienten (88 %) haben bei Aufnahme, stationär und/oder bei Entlassung mindestens ein PIM erhalten. 126 Patienten (63 %) nahmen bei Aufnahme, 168 (84 %) während des stationären Aufenthaltes und 126 (63 %) bei Entlassung PIM ein. Nach FORTA waren es 77 bei Aufnahme, 104 stationär und 70 bei Entlassung, nach PRISCUS 45, 104 und 44 und nach STOPP 100, 123 und 107. Wirkstoffbezogen war der Anteil PIM an der Gesamtmedikation 12,5 % (463 von 3717 Wirkstoffen). 12,7 % (223 von 1757) der Wirkstoffe bei Aufnahme, 11,9 % (397 von 3324) während des stationären Aufenthaltes und 10,5 % (217 von 2068) bei Entlassung waren PIM. Nach FORTA waren es 98 bei Aufnahme, 157 stationär und 88 bei Entlassung, nach PRISCUS 55, 161 und 58 und nach STOPP 163, 224 und 158. Die insgesamt am häufigsten vorkommenden PIM waren Zopiclon (n = 63) (durch die PRISCUS-Bewertung), Acetylsalicylsäure (n = 23) (durch die STOPP-Bewertung), Lorazepam (n = 22) (durch FORTA, PRISCUS und STOPP), Amlodipin (n = 21) (durch die STOPP-Bewertung) und Ibuprofen (n = 20) (durch FORTA und STOPP). Bei Gruppierung nach Level 3 ATC-Code wurden Wirkstoffe aus der Gruppe der Hypnotika und Sedativa mit Abstand am häufigsten angewendet (n = 84). Die Beurteilung eines möglichen Zusammenhanges zwischen PIM und Hauptdiagnose bzw. schwerwiegenden Auswirkungen während des stationären Aufenthaltes wurde nach einer Analyse gemäß dem WHO-Schema zur Kausalitätsbewertung durch-geführt. Es konnte insbesondere für Amlodipin, Lorazepam, NSAR und Risperidon ein Zusammenhang mit der Hauptdiagnose gefunden werden. Diese PIM hatten auch während des stationären Aufenthaltes schwerwiegende Auswirkungen. Solche schwerwiegenden Auswirkungen waren für Amlodipin, Lorazepam und Risperidon beispielsweise Sturz-ereignisse, für die NSAR gastrointestinale Blutungen. Mit Hilfe der gewonnenen Informationen und einem Vergleich mit der Literatur wurde ein praxistaugliches Tool zur Minimierung des PIM-Einsatzes entwickelt. Das Tool GERAS umfasst fünf Punkte: 1. Ansetzen eines neuen Arzneimittels – wenn: „Start low, go slow“, 2. Vorgehen bei einem Sturz unter antihypertensiver Medikation, 3. Hinweise zum Sturzrisiko durch Hypnotika und Psycho-pharmaka, 4. Vorgehen bei Schlafstörungen und 5. wenn NSAR, dann mit PPI. Mit GERAS steht ein neuartiges, einfaches und praxisnahes Tool zur möglichen Reduktion von PIM und damit der Erhöhung der Arzneimitteltherapiesicherheit im Krankenhaus zur Verfügung. GERAS wurde im April 2014 UKE-weit eingeführt und wird seitdem in der täglichen Stationsarbeit von Ärzten, Apothekern und Pflegekräften genutzt.

Elderly patients are the most important target group of pharmacotherapy. Older individuals often suffer from multiple co-morbidities, which often results in polypharmacy. Therapy based on guidelines can be problematic and is rarely examined in clinical trials of elderly patients. In addition, alterations in pharmacokinetics and pharmacodynamics due to increased age of patients have to be considered. As a result of these changes, the elderly are particularly vulnerable to certain drugs. These drugs are classified as potentially inappropriate medication (PIM) for the elderly because they bear an increased risk of adverse drug events and result in major patient safety concerns. Several classifications have been published to identify and avoid PIM. For this study, the three following screening tools were chosen to identify PIM: FORTA (Fit for the Aged), PRISCUS (Latin: time-honoured) and STOPP (Screening Tool of Older Persons’ potentially inappropriate Prescriptions). The aims were to determine which PIM are taken by elderly patients at University Medical Centre Hamburg-Eppendorf (UKE) and how the prevalence of PIM changes from admission to discharge. In addition, a possible association between PIM and primary diagnosis/reason for admission, as well as serious side effects and consequences of PIM use during hospitalization were examined. Using the information gained a practical tool for minimizing PIM use was developed. Based on the criteria provided by FORTA, PRISCUS and STOPP, medication of patients over the age of 65 was screened within four point prevalence analyses at admission, during inpatient stay and at discharge, respectively. Medication was recorded and correlated to diagnoses and reason for admission. Patients were included in the study if they were admitted via the emergency department with at least five drugs prescribed upon admission. The average age was 78.8 years, 60.5 % of the patients were female. 176 of 200 patients (88 %) received at least one PIM at admission, during inpatient stay and/or at discharge. 126 patients (63 %) received at least one PIM at admission, 168 (84 %) received at least one PIM during inpatient stay and 126 (63 %) at discharge. FORTA identified 77 patients as receiving PIM upon admission, 104 during inpatient stay, and 70 upon discharge; PRISCUS identified 45, 104, and 44 patients; and STOPP identified 100, 123, and 107 patients, respectively. Multiple classifications were possible. Concerning drugs, the proportion of PIM of the total medication was 12.5 % (463 of 3717 drugs). 12.7 % (223 of 1757) of drugs at admission, 11.9 % (397 of 3324) during inpatient stay and 10.5 % (217 of 2068) at discharge were PIM. According to FORTA there were 98 at admission, 157 during inpatient stay and 88 at discharge; according to PRISCUS 55, 161 and 58; according to STOPP 163, 224 and 158. The PIM prescribed most often were zopiclone (n = 63) (due to PRISCUS rating), acetylsalicylic acid (n = 23) (due to STOPP rating), lorazepame (n = 22) (due to FORTA, PRISCUS and STOPP), amlodipine (n = 21) (due to STOPP rating) and ibuprofen (n = 20) (due to FORTA and STOPP). When grouped according to level 3 ATC code, drugs from the group of hypnotics and sedatives were by far the most commonly used (n = 84). The evaluation of a possible association between PIM and primary diagnosis or severe effects during hospitalisation was performed according to an analysis using the WHO causality assessment. Particularly for amlodipine, lorazepame, NSAIDs and risperidone, an association with the primary diagnosis was found. These PIM had serious side effects during inpatient stay as well. Serious effects were, for example, falls following amlodipine, lorazepame and risperidone intake or gastrointestinal bleeding following NSAIDs. Due to these PIM and the associated risk, followed by a comparison with the literature, the following tool GERAS (GERiatrische Arzneimitteltherapie-Sicherheit, German: geriatric drug safety) was developed: 1. Prescribing of a new drug – if: „Start low, go slow“, 2. Procedure in case of a fall under antihypertensive medication, 3. Notes on fall risk by hypnotics and psychotropic drugs, 4. Procedure by insomnia and 5. if NSAIDs, then with PPI. GERAS is a novel, simple and practical tool to minimize PIM-intake during hospital stay. It is possible to improve the safe use of medication for elderly people. GERAS was introduced in UKE in April 2014 and is now used by clinical pharmacists, physicians and nurses in daily routine.
