Zusammenfassung: 
Der Gesamt-DNA-Gehalt von Karzinomzellen (Ploidiestatus) wurde in den 80er und 90er Jahren des letzten Jahrhunderts als revolutionärer diagnostischer Prognosemarker beim Prostatakarzinom diskutiert. Obwohl die Studienlage hierzu bis heute unklar ist, gibt es weiterhin Institutionen, welche die Anwendung der DNA-Zytometrie für die Wahl der Therapie propagieren.
Ziel dieser Arbeit war es, die mögliche klinische Relevanz der DNA-Zytometrie endgültig zu klären.
Untersucht wurden 1.038 Prostatakarzinome mittels Flowzytometrie. In 60% wurde ein diploider, in 38% ein tetraploider und in 2% ein aneuploider Befund festgestellt.
Der Ploidie-Status korreliert signifikant mit dem Gleason-Grad und dem pT-Stadium. Ein diploider Ploidie-Status fand sich in 70% der Gleason ≤3+3=6, in 64% der Gleason 3+4=7, in 50% der Gleason 4+3=7 und in 35% der Gleason ≥4+4=8 Tumoren (p<0.0001). Ein diploider Ploidie-Status fand sich in 66% der pT2, 55% der pT3a und 37% der pT3b/pT4 Karzinome (p<0.0001). Der Ploidie-Status war auch signifikant mit dem pN-Status vergesellschaftet (p=0.0196).
Die Untersuchung der Beziehung zwischen Ploidie-Status und dem PSA-freien Überleben ergab ein signifikant schlechteres Überleben bei tetraploiden und aneuploiden Tumoren als bei diploiden, wenn alle Tumoren gemeinsam analysiert wurden (p=0.0020). Die separate Untersuchung von 195 Gleason ≤3+3=6 (p=0.8667), 503 Gleason 3+4=7 (p=0.9918), 177 Gleason 4+3=7 (p=0.1250) und 87 Gleason ≥4+4=8 (p= 0.7876) ergab jeweils keine signifikanten Unterschiede zwischen dem Ploidiestatus und der Prognose. Dementsprechend ergab die multivariate Analyse keine unabhängige prognostische Bedeutung der Ploidie.
Zusammengefasst ergeben die Untersuchungen dieser Studie, dass ein aberranter Ploidiestatus eine signifikante, aber insgesamt dennoch geringe prognostische Bedeutung aufweist, welche in multivariaten Analysen bzw. Untersuchungen von Tumoren mit identischem Gleason-Grad keine unabhängige prognostische bzw. zusätzliche prognostische Relevanz besitzt. Außerdem wurde in mehr als 35% aller Gleason ≥ 4+4 Karzinome ein diploider Befund erhoben.
Eine klinische Anwendung der Ploidie-Messung beim Prostatakarzinom ist daher nicht indiziert.

The total DNA content of carcinoma cells (ploidy) was discussed in the 80s and 90s of the last century as a revolutionary diagnostic prognostic marker in prostate cancer. Although the status of this study today is not clear, there are still institutions which promote the use of the DNA-cytometry for their choice of therapy.
The aim of this study was to clarify the potential clinical relevance of DNA cytometry conclusively.
1.038 prostate cancers were examined by flow cytometry. Findings of 60% diploid, 38% tetraploid and 2% of aneuploid were detected.
The ploidy status was significantly correlated with the Gleason grade and pT stage. A diploid ploidy status was found in 70% of Gleason ≤3+3=6, 64% of Gleason 3+4=7, 50% of Gleason 4+3=7 and in 35% of Gleason ≥4+4=8 tumors (p <0.0001). A diploid ploidy status was found in 66% of pT2, 55% of pT3a and 37% of the pT3b/pT4 carcinomas (p<0.0001). The ploidy status was also significantly socialised with the pN status (p=0.0196).
The study of the relationship between ploidy status and the PSA-free survival rates gave a significantly poorer survival rating in tetraploid and aneuploid tumors than in diploid when all the tumors were analyzed together (p=0.0020). The separate study of 195 Gleason ≤3+3 =6 (p =0.8667), 503 Gleason 3+4=7 (p=0.9918), 177 Gleason 4+3=7 (p =0.1250) and 87 Gleason ≥4+4=8 (p =0.7876) revealed no significant differences between the ploidy status and the prognosis. Accordingly, the multivariate analysis revealed no significant argument for ploidy.
In summary, these investigations into the study found that aberrant ploidy status has a marked, but overall still low prognostic significance, which has no independent prognostic or additional prognostic value in multivariate analysis respectively in studys of tumours with an identical Gleason grade. In addition, a raised result in diploid was found in more than 35% of all Gleason ≥4+4 carcinomas.
A clinical use of ploidy reading in prostate cancer is therefore not indicated.
