Zusammenfassung: 
Noni – Saft, der aus den Früchten der Pflanze Morinda citrifolia L. gewonnen wird, ist ein pflanzliches Mittel, dass seit Jahrtausenden als Heilmittel gegen die unterschiedlichsten Erkrankungen eingesetzt wird. Mehrere der traditionellen Indikationen konnten mittlerweile experimentell bestätigt werden. Dazu gehören u.a. die entzündungshemmende und immunstimulierende Wirkung.
Schmerzen sind ein empfundenes unangenehmes Gefühl, das im Rahmen einer Entzündung auftreten kann. Entzündungsreaktionen und damit auch Schmerzen werden durch Cortisol supprimiert.
In vier unterschiedlichen Pilotstudien wurden die Wirkungen von Noni-Saft untersucht.
Es wurde der Zytokinverlaufsspiegel im Blutserum am Menschen gemessen sowie parallel dazu Schmerzverlauf und Wohlbefinden dokumentiert. Des Weiteren folgte ein Pilotversuch zur Wirkung von Noni auf den Cortisolspiegel. Als viertes wurde die zentral und peripher schmerzstillende Wirkung von Noni an Mäusen getestet.

Es konnte gezeigt werden, dass Noni im Tierversuch eindeutig zu einer Hemmung des Schmerzempfindens führte, sowohl peripher als auch im zentralen Nervensystem. Es verbesserte bei chronischen Schmerzpatienten das Wohlbefinden bei regelmäßiger Einnahme über einen längeren Zeitraum. Zusätzlich erhöhte es den Cortisolspiegel, was entzündungshemmend und damit schmerzlindernd wirken kann.
Die Entzündungs-Zytokine zeigten häufig einen initialen Anstieg, dem ein Abfall auf oder unter den Anfangsspiegel folgte. Wegen der begrenzten Anzahl der untersuchten Patienten konnte dieser Befund aber nicht statistisch belegt werden. Ebenso blieben wir den Beweis für die Verringerung der subjektiven Schmerzwahrnehmung schuldig.
Es müssen weitere - deutlich größere - Studien veranlasst werden, um vor allem die Aussagen beim Menschen quantitativ ausreichend zu erfassen.

Noni juice, which is prepared from the fruits of the tropical tree Morinda citrifolia, is an herbal remedy used traditionally in the South Pacific for multiple disorders. Several of these traditional indications have been confirmed by experimental and clinical investigations during the last decade. Among these are the immunstimulating and anti-inflammatory activity of Noni.

Pain is an uncomfortable feeling occurring during inflammative reactions. Inflammation and thus also pain are reduced by excretion of cortisol.
We investigated the activity of Noni juice in four independent pilot experiments. The cytokine level in the blood serum of patients with chronic pain was monitored together with an individual score of pain and overall behaviour. A pilot study on the cortisol and ACTH level after intake of a bolus of Noni juice was additionally invesitigated in one patient. Finally we tested the antinociceptive acitivity of Noni juice in mice.
The hot plate test with mice resulted in a statistical significant increase of the pain threshold after oral treatment of the mice with 10% Noni juice in their drinking water. The activity of Noni juice was comparable to that of the central analgesic active compound tramadol, but in contrast to this drug it could only be partially inhibited by the opioid antagonist naloxon. This indicates that both, peripheral and central mechanisms are involved in the the analgesic activity of Noni juice.
In patients with chronic pain, Noni juice increased the overall well being if taken over a period of several weeks. It could also be demonstrated that the juice did increase the cortisol and ACTH level after bolus application, which was followed by an anti-inflammatory and analgesic effect. Proinflammatory cytokines, such as IL-6, increased initially after ingestion of Noni juice but decreased thereafter. Because of the limited number of patients involved in this study, no statistical significance could be demonstrated for this effect. The subjective reduction of the pain score of patients with chronically pain could also not be evaluated significantly. Further studies are necessary in order to better demonstrate the analgesic and anti-inflammatory activity of Noni juice in humans qualitatively and quantitatively.
