Zusammenfassung: 
Hintergrund: Der 1991 von der EuroQoL Arbeitsgruppe entwickelte Fragebogen EQ-5D™ zur gesundheitsbezogenen Lebensqualität (HRQoL) mit nur 5 Fragen verschiedener Funktionsbereiche (Deskriptives System) und einer visuellen Analogskala (VAS) findet weltweit sehr breite Anwendung. Die 2011 von der EuroQol Arbeitsgruppe aus dem EQ-5D-3L™ weiterentwickelte Version EQ-5D-5L™ beinhaltet die Erweiterung der Antwortskala des deskriptiven Systems von 3 auf 5 Kategorien, die Validierung ist weltweit noch im Prozess. Bisher gibt es keine Ergebnisse einer Validierung des EQ-5D-5L™ für Patientinnen und Patienten mit Herzinsuffizienz in Deutschland.
Material/Methoden: Die Baseline-Daten der Patientinnen und Patienten mit Herzinsuffizienz der RECODE-HF Studie werden aufbereitet und explorativ ausgewertet. 3225 Fälle mit vollständigem EQ-5D-5L™ Deskriptivem System werden für die konfirmatorische Faktorenanalyse (CFA) selektiert. Standardkriterium zur Beurteilung des Schweregrades der Herzinsuffizienz ist die New York Heart Association- (NYHA-) Klassifikation. Die diskriminative Validität wird für die 5 EQ-5D-5L™ Fragen (F1 bis F5) mit dem Pearson Chi-Quadrat Test und für die EQ-5D-5L™ VAS und Indexwerte (deutscher und UK Index; Cross-Walk Kalkulation) mit univariaten Varianzanalysen (n = 3110 bzw. n=3171) geprüft und die Effektstärken durch Pearson‘s r und das partielle Eta-Quadrat beschrieben. Das CFA Basismodell zur Vorhersage des Konstruktes Lebensqualität wird mit den Methoden Maximum- Likelihood (ML) und asymptotisch verteilungsfreie Schätzer (ADF) berechnet. Zur Prüfung der Einflussfaktoren Körperliche Aktivität und Depression wird ein CFA Mediatormodell mit ML Methode genutzt. Das CFA Basismodell wird für die reduzierte Fallzahl aus dem Mediatormodell wiederholt (n = 3064). Für die Konstruktanalyse der EQ-5D-5L™ VAS und der EQ-5D-5L™ Indexwerte wird der Zusammenhang mit der assoziierten Messung SF-36 Allgemeine Gesundheit (n = 3155 und n = 3210) sowie den Einflussfaktoren (n = 3015 und n = 3064) anhand univariater Varianzanalysen getestet und die Effektstärken beschrieben. Die Analysen erfolgen mit IBM SPSS / IBM SPSS AMOS 23.
Ergebnisse: Für alle 3 EQ-5D-5L™ Parameter ist der Vergleich nach NYHA-Stadium statistisch auffällig (P < 0,001; F1, F3, VAS, Indizes: Effekt mittel; F2, F4, F5: klein). Für das EQ-5D-5L™ Deskriptive System zeigen sich wenige Unterschiede zwischen Stadium I und II, jedoch Veränderungen im Stadium III und IV. Im CFA Basismodell sind die Faktorreliabilität und durchschnittlich erfasste Varianz akzeptabel (ML 0,87; 0,58; ADF 0,87; 0,59). Der globale Fit variiert (Chi²; p; Chi²/df; TLI; CFI; RMSEA) mit ML 151,40; 0,000; 30,28; 0,96; 0,98; 0,095; mit ADF 110,56; 0,000; 22,11; 0,89; 0,94; 0,081. Die standardisierten Ladungen der EQ-5D-5L™ Fragen 1-5 auf das Konstrukt Lebensqualität sind mit ML 0,81; 0,76; 0,90; 0,69; 0,51; mit ADF 0,82; 0,76; 0,90; 0,71; 0,51. Die Varianzaufklärung für F1 bis F4 ist gut (0,68; 0,57; 0,81; 0,50), für F5 nicht akzeptabel (0,26); mit ML 0,65; 0,56; 0,82; 0,48 und 0,26. Das ML Mediatormodell zeigt einen kleinen Effekt körperlicher Aktivität (r = 0,27) und einen großen negativen Depressionseffekt (r = -0,50) auf HRQoL. Die Mediatoren korrelieren untereinander (-0,17). Das zweite ML Basismodell bestätigt die Ergebnisse. Die Konstrukt- einschließlich der Mediatoranalysen der EQ-5D-5L™ VAS und Indizes sind statistisch auffällig (P < 0,001; SF-36 Allgemeine Gesundheit: großer Effekt; Körperliche Aktivität jeweils kleiner Effekt; Depression jeweils großer Effekt).
Schlussfolgerungen: Alle 3 Parameter des EQ-5D-5L™ grenzen sich nach NYHA-Stadium zuverlässig voneinander ab und und zeigen eine akzeptable diskriminative Validität bei Herzinsuffizienz. Das EQ-5D-5L™ Deskriptive System misst das Konstrukt der HRQoL bei Herzinsuffizienz akzeptabel und die Konstruktanalysen der EQ-5D-5L™ VAS und Indizes zeigen einen starken Zusammenhang mit dem SF-36 Allgemeinen Gesundheitszustand. Körperliche Aktivität und Depression werden in der Anwendung des EQ-5D-5L™ bei Herzinsuffizienz als Einflussfaktoren für die HRQoL bestätigt. Zusammenfassend kann angenommen werden, dass der Einsatz des EQ-5D-5L™ zur Einschätzung der HRQoL für Patientinnen und Patienten mit Herzinsuffizienz geeignet ist.

Background: Developed from the EuroQol Group in 1991 as a short questionnaire for health-related Quality-of-life (HRQoL) with only 5 questions of different functional areas (Descriptive System) and a visual analog scale (VAS), the EQ-5D™ is widely used worldwide. The EQ-5D-5L™ version, further developed by the EuroQol Group from the EQ-5D-3L™ in 2011, includes the expansion of the descriptive system response scale from 3 to 5 categories, and the validation is still in progress worldwide. So far, there are no results of validation of the EQ-5D-5L™ for patients with heart failure (HF) in Germany.
Material/Methods: Baseline data of patients of the RECODE-HF study have been processed and exploratively evaluated. A total of 3225 cases with fully answered EQ-5D-5L™ Descriptive System is available for confirmatory factor analysis (CFA). The severity of the New York Heart Association (NYHA) classification is the standard criterion for assessing HF. Discriminant validity is calculated for the 5 EQ-5D-5L™ questions (F1 to F5) with Pearson’s chi-squared test, and for the EQ-5D-5L™ VAS and index (German and UK index value by cross-walk calculation) univariate variance analysis are calculated, and effect sizes described by Pearson’s r and partial eta squared. A basic model for prediction of the construct validity of HRQoL by F1 to F5 is developed by means of CFA with maximum likelihood (ML) and asymptotically-distribution-free (ADF) method. A CFA mediator model (ML) is used to verify the influence of the factors physical activity and depression. The CFA basic model is repeated for the reduced number of cases from the mediator model (n = 3064). For construct analysis of the EQ-5D-5L™ VAS and the German and UK EQ-5D-5L™ index, the correlation with the associated measurement of SF-36 General Health (n = 3155 and n = 3210 respectively) as well as the influence factors (n = 3155 and n = 3210 respectively) is calculated by means of univariate variance analyses, and effect sizes are given. The analyzes are calculated with IBM SPSS / IBM SPSS AMOS 23.
Results: For all three EQ-5D-5L™ parameters, the comparison with the NYHA group is statistically noticeable (p <0.001; F1, F3, VAS, indices: effect moderate; F2, F4, F5: small). For the EQ-5D-5L™ descriptive system there are few differences between stage I and II, however, changes in stage III and IV become apparent. In the CFA basic model, factor reliability and average variance are acceptable (0,87; 0,59; ML 0,87; 0,58). The global fit varies (Chi²; p; Chi²/df; TLI; CFI; RMSEA) with ML 151,40; 0,000; 30,28; 0,96; 0,98; 0,095; with ADF 110,56; 0,000; 22,11; 0,89; 0,94; 0,081. The standardized regression coefficients of F1 to F5 on the construct HRQoL are with ML 0,81; 0,76; 0,90; 0,69; 0,51; with ADF 0.82; 0,76; 0,90; 0,71; 0,51. The explained variance of F1 to F4 is good (ML 0,65; 0,56; 0,82; 0,48), for F5 not acceptable (0,26); with ADF 0,68; 0,57; 0,81; 0,50, and 0,26. The ML mediator model shows a small effect of physical activity (r = 0,27), a strong negative effect of depression (r = -0,50), and a strong correlation of physcial activity with depression (r = -0,17). The second ML basic model confirms the results. Construct, including mediator analyses for the EQ-5D-5L™ VAS and indices are statistically noticeable (P < 0,001; SF-36 General health: strong effect; physcial activity small effect each; depression strong effect each).
Conclusions: All 3 parameters of the EQ-5D-5L™ are reliably separated from each other according to the NYHA stage and show an acceptable discriminative validity in patients with heart failure. The EQ-5D-5L™ Descriptive System measures the construct of HRQoL acceptable, and construct analyses of the EQ-5D-5L™ VAS and indices show a strong correlation with the SF-36 General health. Physical activity and depression are confirmed by the use of the EQ-5D-5L™ as factors influencing HRQoL in patients with HF. In summary, it can be assumed that the use of the EQ-5D-5L™ is suitable for the assessment of HRQoL in patients with heart failure.
