Das duktale Adenokarzinom des Pankreas (PDAC) ist inzwischen die dritthäufigste tumorbedingte Todesursache in den USA. Zur Entwicklung innovativer Therapiestrategien ist ein besseres molekularbiologisches Verständnis der Tumorprogression essentiell.
In der vorliegenden Arbeit wurde die prognostische Relevanz von Hyaluronan und ITGAV untersucht. Der Anteil an Hyaluronan in der Matrix des PDACs erreicht keine prognostische Signifikanz, während ITGAV positive Tumore mit einem verminderten Überleben assoziiert sind. ITGAV eignet sich somit als prognostischer Marker für das PDAC.
Im intraperitonealen Xenograft-Mausmodell führte der Knockdown von ITGAV zu einer Reduktion der Peritonealkarzinose. In Versuchstieren mit Knockout der E- und P-Selektinen vermindert der Knockdown von ITGAV die Peritonealkarzinose zusätzlich. Das subkutane Xenograft-Mausmodell bestätigt die ITGAV vermittelte Tumorprogression. Primärtumorwachstum und Fernmetastasierung nehmen unter dem Einfluss von ITGAV zu. In vitro vermittelt ITGAV Migration, Proliferation und Adhäsion der Tumorzellen.
Es wurde nachgewiesen, dass ITGAV latentes TGF-β im PDAC aktiviert. Mechanistisch beruhen die Effekte des ITGAV Knockdowns somit auf dem Verlust des Integrins selbst sowie des Integrin eigenen Signaling und zum anderen aus der verminderten Aktivität der TGF-β Signalkaskade.
Die Inhibition vom ITGAV ist eine neue vielversprechende Therapiestrategie für das duktalen Adenokarzinoms des Pankreas.

For the first time in 2016, pancreatic ductal adenocarcinoma (PDAC) was the third most common tumor-associated cause of death in the USA. A better understanding of not only the molecules involved in progression at the molecular level, but also the resulting innovative therapeutic approaches is urgently needed to improve the clinical situation. The tumor stroma represents a barrier to the diffusion of chemotherapeutic agents and biologicals meaning that substances do not reach all the tumor cells. Hyaluronan is a major component in this extracellular matrix of the tumor stroma. Interaction with this matrix is regulated by the cell-matrix adhesion molecule integrin alpha V (ITGAV). Changes in cell-matrix interaction are functionally important since they enable the tumor cells to invade the adjacent tumor stroma.
This study investigated the relevance of hyaluronan and ITGAV to prognosis of patients with PDAC using tissue microarray. The level of hyaluronan in the PDAC matrix did not have prognostic significance, while ITGAV-positive tumors were associated with a lower rate of survival. ITGAV is thus suitable as a prognostic marker for PDAC.
In the intraperitoneal xenograft mouse model, ITGAV knockdown led to a reduction of peritoneal carcinomatosis. In E-selectin and P-selectin knockout mice the ITGAV knockdown reduced the peritoneal carcinomatosis in an additive manner. The subcutaneous xenograft mouse model, where primary tumor growth and distant metastases increase under the influence of ITGAV, confirms ITGAV-mediated tumor progression. In vitro ITGAV regulates migration, proliferation and adhesion of tumor cells
In respect to the underlying mechanism, it was shown that ITGAV activates latent TGF-β in PDAC. Subsuming the effects of ITGAV knockdown are based on the loss of the integrin itself and its own signaling, as well as on the reduced activity of the TGF-β signaling pathway.
ITGAV inhibition is a new and promising therapeutic strategy for pancreatic ductal adenocarcinoma. How to effectively inhibit ITGAV pharmacologically in PDAC emerges from this investigation as a new scientific challenge.
