Zusammenfassung: 
Sowohl extrakorporale Systeme zur Kreislaufunterstützung (ECLS) als auch ein gezieltes Temperaturmanagement mit 32 °C bis 34 °C stellen vielversprechende Therapiealternativen zur Verbesserung der neurologischen Prognose und der weiterhin hohen Mortalität nach Reanimation dar. Dennoch wird vor dem gleichzeitigen Einsatz im klinischen Alltag aus Sorge vor vermehrten Blutungskomplikationen unter Hypothermie weiterhin häufig zurückgeschreckt. Die vorliegende retrospektive monozentrische Erhebung untersuchte daher erstmalig umfassend und systematisch anhand eines Kollektivs von 78 Patienten aus den Jahren 2009 bis 2015 den möglichen Einfluss einer milden therapeutischen Hypothermiebehandlung auf die Häufigkeit von Blutungskomplikationen unter ECLS innerhalb der ersten 72 Stunden nach Reanimation. Blutungen stellten dabei auch in der vorliegenden Erhebung eine der häufigsten beobachteten Komplikationen bei Patienten unter extrakorporaler Kreislaufunterstützung dar. Mit einer Inzidenz von 92,9 % in der Normothermiegruppe und 86,1 % in der Hypothermiegruppe kam es jedoch auch unter Hypothermiebehandlung weder zu Häufung der gesamten Blutungskomplikationen, noch zu einem erhöhten Anteil der prognostisch relevanten Majorblutungen. Letztere waren mit einem Anteil von 46,1% in der Normothermie bzw. 43,2% in der Hypothermiegruppe ebenfalls in beiden Gruppen gleich hoch. Selbst bestehende Majorblutungen konnten unter Hypothermie innerhalb kurzer Zeit unter Kontrolle gebracht werden und darüber hinaus trotz erhöhter Morbidität in der Hypothermiegruppe eine vergleichbare Überlebensrate wie in der weniger kranken Normothermiegruppe erreicht werden. Die Ergebnisse sprechen somit in vielen Punkten dafür, dass die sichere Anwendung einer milden therapeutischen Hypothermiebehandlung unter ECLS selbst bei bestehenden Blutungskomplikationen möglich sein könnte. Damit würde die Erhebung zukünftig zu einer fundierten Entscheidungsfindung beitragen können, ob selbst bei vorbestehender Blutungsneigung oder bestehenden Blutungen dennoch eine milde therapeutische Hypothermiebehandlung unter ECLS durchgeführt werden sollte, um auch für diese Patienten die zusätzlichen prognoseverbessernden Effekte dieser Behandlung zu erhalten.

Both, Extracorporeal Life Support Systems (ECLS) and targeted temperature management at 32 °C to 34 °C are two highly promising therapy approaches for improving the neurological prognosis and reducing the high mortality and morbidity due to cardiac arrest. Many clinicians, however, remain reluctant to simultaneously apply ECLS and a targeted temperature management due to concerns about increased bleeding complications associated with mild therapeutic hypothermia. The present retrospective single-centre study with an observation period from 2009 to 2015, based on 78 patients with cardio-pulmonary resuscitation and ECLS for the first time systematically and comprehensively further investigates the possible influence of a combined treatment with mild therapeutic hypothermia and ECLS on the frequency of bleeding complications within the first 72 hours after cardio-pulmonary resuscitation. In this study, despite of haemorrhagic complications ranging among the most frequent complications during ECLS, there was nevertheless no evidence of an increased bleeding propensity in the group of patients with hypothermia treatment neither with regard to bleeding of any severity nor for the category of prognostically highly relevant major bleeding events. With an incidence of 92,9 % under normothermic treatment and 86,1 % under treatment with mild therapeutic hypothermia there was no significant difference in the appearance of bleeding of all severities. Also, major bleedings with 46,1% under normothermia and 43,2% under targeted temperature management respectively occurred with equal frequencies in both groups. Even pre-existing major bleedings could be brought under control in the case of therapeutic hypothermia. In addition, despite higher morbidity in the hypothermia group, the survival rate was still comparable to the group of less ill patients with a normothermic treatment. The results of this study therefore suggest that a safe application of therapeutic hypothermia and ECLS might be possible, even in case of pre-existing bleeding complications. Thus, this study could contribute to an informed decision-making as to whether mild hypothermia treatment under ECLS should be performed even in patients with pre-existing bleeding propensity or existing bleeding for taking advantage of the neuroprotective effects of a targeted temperature management for those patients as well.
