| de |
| dc.description.abstract | Background: Carcinomas of the nasal cavity and paranasal sinuses are rare malignant diseases. The data regarding these tumors is highly limited and uncertain. Early symptoms are often nonspecific, which frequently leads to delay in diagnosis. Despite the wide variability in tumor entities, nasal and paranasal sinus carcinomas are often treated similarly to head and neck tumors due to the lack of robust data. Objective: The aim of this study was to analyze the epidemiological, histopathological, and symptomatic characteristics, as well as the diagnostics, therapy, and clinical course of patients with nasal or paranasal sinus carcinomas treated at the Universitätsklinikum Hamburg-Eppendorf (UKE). The study specifically focused on the duration of symptoms before diagnosis and the type of initial symptoms, in order to derive insights for early detection. Methods: A retrospective analysis from 158 patients with nasal and paranasal sinus carcinoma who presented to the Department of Otorhinolaryngology at UKE between 2008 and 2021 was conducted. Electronic medical records were searched for specific variables and statistical analysis including correlation analyses were executed. Results: 55.7% of the patients were male and 44.3% female. The average age was 59 years. The most common tumor entity was squamous cell carcinoma, accounting for 56.33%, followed by malignant melanoma (8.23%) and esthesioneuroblastoma (6.96%). 60.28% were diagnosed at an advanced tumor stage (T3-T4). The average duration from symptom onset to diagnosis was 6.22 months. The most common symptoms at presentation were pain (33.6%), epistaxis (32.1%) and nasal obstruction (30.6%). The presence of ophthalmological symptoms was associated with orbital invasion and advanced tumor stage. At the time of diagnosis, 16.5% of patients already had more than one carcinoma. The incidence at UKE increased from 2008–2012 but remained constant until 2021. Most patients (82.58%) initially underwent surgical treatment. Additionally, 44.1% received adjuvant therapy. The higher the tumor stage, the lower the remissionrate. During the clinical course, 39% experienced a local recurrence, and 21.4% developed distant metastases. Conclusion: Nasal and paranasal sinus carcinomas typically present at advanced stage and the time from onset of initial symptoms to diagnosis is comparatively long. Symptoms are often nonspecific, which leads to delayed diagnosis and thus worsens the patients’ prognosis. This systematic analysis of initial symptoms provides new insights into improving early detection, which should be further investigated in future prospective studies. | en |
