dc.description.abstractDie vorliegende Arbeit untersucht die Effekte einer integrativen Intervention in der stationären Rehabilitation von Substanzabhängigen mit komorbider PTBS (N=539) auf die Outcomes PTBS-Symptomatik, Selbstfürsorge und Lebensqua-lität. Für die Untersuchung wurden nacheinander eine Kontroll- und eine Inter-ventionsgruppe in fünf Fachkliniken rekrutiert und behandelt. Hierbei gab es nach einem Screening auf PTBS (T0) drei Testzeitpunkte: bei Aufnahme (T1), bei Ent-lassung (T2; 3 Monate nach Aufnahme) und postalisch sechs Monate nach Ent-lassung (T3). Beide Gruppen erhielten das gängige Behandlungsprogramm der jeweiligen Klinik („Treatment as usual“). In der Interventionsgruppe wurde zusätz-lich die kognitiv-behaviorale Therapie „Sicherheit finden“ mit maximal 12 wö-chentlichen Sitzungen in halboffenen Gruppen eingesetzt. Hypothese: Signifikant bessere Outcomes in der Interventionsgruppe in Bezug auf die Ausprägung der PTBS-Symptomatik sowie die Selbstfürsorge und die Lebensqualität zu den Testzeitpunkten T2 und T3. In Bezug auf die PTBS-Symptomatik, ermittelt mithilfe der Posttraumatic Diag-nostic Scale, zeigte sich hypothesenkonform ein positiver Effekt in der Interven-tionsgruppe zum Zeitpunkt T2. Die PTBS-Symptomatik fiel demnach in der Inter-ventionsgruppe signifikant geringer aus, als in der Kontrollgruppe. Zum Zeitpunkt T3 war dieser Effekt jedoch nicht mehr zu beobachten. In Bezug auf den Out-come Selbstfürsorge fand sich anhand des Hamburger Selbstfürsorge Fragebo-gens lediglich ein statistischer Trend zum Zeitpunkt T2 für die Subskala Pacing (achtsamer Umgang mit der eigenen Person und ihren Grenzen). Darüber hinaus konnte kein Effekt auf die Selbstfürsorge festgestellt werden. In Bezug auf die Lebensqualität anhand des 12-Item Short-Form Health Survey fand sich eine leicht niedrigere Lebensqualität unter den Teilnehmern der Interventionsgruppe zum Zeitpunkt T3. Es konnte festgestellt werden, dass zum Zeitpunkt T2 eine Korrelation zwischen der Anzahl an Interventionssitzungen und der PTBS-Symptomatik bestand. Mehr Therapiesitzungen korrelierten in der Interventionsgruppe mit einer geringeren PTBS-Symptomatik. Weiter konnte festgestellt werden, dass es zum Zeitpunkt T3 eine Korrelation zwischen der Ausprägung der PTBS-Symptomatik und der Abstinenz gab. Eine geringere PTBS-Symptomatik korrelierte hier mit einer erhöhten Wahrscheinlich-keit für ein abstinentes Verhalten, berechnet über beide Gruppen. Die Ergebnisse legen nahe, dass therapeutische Angebote in Bezug auf die PTBS-Symptomatik integriert in die Suchtbehandlung sinnvoll sind. Zum einen konnte gezeigt werden, dass sich die PTBS-Symptomatik durch die Interventio-nen mindern ließ. Zum anderen zeigte sich ein deutlicher Zusammenhang zwi-schen der PTBS-Ausprägung und abstinentem Verhalten. Da die Effekte auf die PTBS-Symptomatik sechs Monate nach Entlassung aus dem stationären Aufent-halt nicht mehr signifikant waren, wird die Bedeutung einer gezielten ambulanten Nachbehandlung deutlich, damit Effekte der Therapie länger aufrechterhalten werden können.de
dc.description.abstractThis dissertation examines the effects of an integrative intervention on the out-comes PTSD symptomatology, self-care and quality of life within the clinical re-habilitation process of substance abusing patients with comorbid PTSD (N=539). For this study, both a control group and an intervention group were recruited and treated in five specialised clinics. After an initial screening for PTSD (T0) there were three further checkpoints for testing: at admission (T1), upon discharge (T2; three months after admission) and via mail six months after discharge (T3). Both groups received treatment as usual at their respective clinics. In addition, the in-tervention group received the cognitive-behavioural therapy “Seeking safety”, consisting of 12 weekly half-open group sessions. Hypothesis: significantly better outcomes at checkpoints T2 and T3 within the intervention group in terms of the extent of the PTSD symptomatology, self-care and quality of life. Regarding the PTSD symptomatology, evaluated with the Posttraumatic Diag-nostic Scale, a positive effect for the intervention group at checkpoint T2 arose, in accordance with the hypothesis. According to the results, the PTSD sympto-matology was significantly lower within the intervention group than within the con-trol group. However, this effect was not detectable anymore at checkpoint T3. In terms of the outcome self-care, evaluated with the Hamburger Selbstfürsorge Fragebogen (Hamburg Self-care Survey), merely a statistical trend arose at checkpoint T2 for the subscale item ‘pacing’ (mindful self-care of one’s own per-son and boundaries). Other than that, no effect on self-care could be established. Regarding quality of life, evaluated with the 12-Item-Short-Form Health Survey, a slightly lower quality of life within the participants of the intervention group at checkpoint T3 was found. At checkpoint T2 it was established that there was a correlation between the amount of intervention sessions and the PTSD symptomatology. More therapy sessions correlated with lower PTSD symptomatology within the intervention group. Furthermore, it was established that there was a correlation between the PTSD symptomatology and the abstinence at checkpoint T3. Lower PTSD symptomatology correlated with a higher probability for abstinence, calculated across both groups. The results suggest the merit of therapeutic offers regarding the PTSD sympto-matology integrated into addiction treatment. On the one hand, it could be shown that the interventions can lower PTSD symptomatology. On the other hand, a significant correlation between PTSD symptomatology and abstinence has been shown. As the effects on the PTSD symptomatology ceased being significant six months after discharge from the clinic, the importance of a targeted, ambulant aftercare is highlighted, to retain the effects of the therapy for a longer period of time.en
