Die chronische Infektion mit dem HBV stellt mit weltweit rund 250 Millionen infizierten Menschen ein bedeutendes globales Gesundheitsproblem dar. Während die Prävention mithilfe eines Impfstoffes möglich ist, gibt es aktuell keine therapeutischen Maßnahmen für eine komplette Heilung. Eine mögliche Behandlungsoption bildet die Gabe von IFN-alpha mit dem Vorteil einer begrenzten Behandlungsdauer und einer anschließenden möglichen Langzeitkontrolle des Virus. Bei IFN-alpha handelt es sich um ein Zytokin, welches in der Etablierung und Regulation einer Immunantwort eine entscheidende Rolle spielt und eine supprimierende Wirkung auf das HBV ausübt. Der genaue Mechanismus dieser Suppression ist jedoch bislang nicht vollständig verstanden. Neben IFN-alpha existieren mit IFN-lambda und IFN-gamma weitere Interferone, die eine ähnliche Signalkaskade aktivieren und einen supprimierenden Effekt auf das HBV ausüben können. Ziel dieser Arbeit war es, mögliche Gemeinsamkeiten und Unterschiede einer direkten antiviralen Wirksamkeit von IFN-lambda und IFN-gamma auf das HBV im Vergleich zu IFN-alpha in vivo im humanisierten uPA/SCID-Mausmodell zu analysieren um dadurch weitere Einblicke in die Wirkung von IFN-alpha zu erlangen. Im Vergleich von pegyliertem IFN-alpha mit pegyliertem IFN-lambda konnte gezeigt werden, dass sowohl PEG-IFN-alpha als auch PEG-IFN-lambda ISGs in einem vergleichbaren Maße in humanen Hepatozyten induziert. Eine deutliche und langfristige Suppression von HBV durch Reduktion serologischer und intrahepatischer viraler Parameter konnte jedoch nur durch PEG-IFN-alpha erreicht werden. Diese Ergebnisse deuten darauf hin, dass die Induktion von ISGs für eine anhaltende und starke antivirale Wirkung auf das HBV in humanisierten uPA/SCID-Mäusen nicht auszureichen und PEG-IFN-alpha einen weiteren Mechanismus zu nutzen scheint. Im Gegensatz zur Behandlung mit PEG-IFN-lambda konnte durch eine Behandlung mit PEG-IFN-alpha der wirtseigene Faktor SMC5/6-Komplex in den Hepatozyten wieder detektiert werden. Dieser Komplex scheint als viraler Restriktionsfaktor die Transkription der viralen RNAs von der cccDNA zu inhibieren. Er wird jedoch in der HBV-Infektion durch das HBx-Protein degradiert, sodass er nicht mehr wirken kann. Die Detektion des Komplexes nach Behandlung mit PEG-IFN-alpha spricht dafür, dass PEG-IFN-alpha, anders als PEG-IFN-lambda, zu einer Reduktion des HBx-Proteins führt, was damit von zentraler Bedeutung für die antivirale Wirksamkeit von PEG-IFN-alpha zu sein scheint. Für zugrunde liegende Mechanismen dieser Reduktion sind jedoch weitere Untersuchungen notwendig. In ersten vergleichenden Analysen der kurzfristigen Wirkung von IFN-gamma und IFN-alpha konnte eine ähnliche Wirksamkeit beider Interferone nach einmaliger Gabe beobachtet werden. Für fundierte Aussagen zur antiviralen Wirksamkeit von IFN-gamma im Vergleich zu IFN-alpha, sollten die beobachteten Effekte jedoch mit einer größeren Stichprobe wiederholt werden und eine längerfristige Gabe der Interferone erfolgen.

Chronic infection with HBV is a global health problem affecting approximately 250 million people worldwide. While a prophylactic vaccine is available, there are currently no therapeutic options to completely cure the disease. Treatment with IFN-alpha is one possible treatment strategy and has the advantage to be of finite duration and the potential to induce sustained off-treatment response at least in a minority of chronically infected patients. Besides being one of the main cytokines responsible for the establishment and regulation of immune responses, IFN-alpha exhibits a suppressing effect on HBV. Knowledge about the exact mechanism of this suppression, however, is still limited. Apart from IFN-alpha, there are two additional Interferons – namely IFN-lambda and IFN-gamma – activating a similar intracellular signaling pathway and having a possible suppressing impact on HBV. Aim of this study was to analyze possible similarities and differences in direct antiviral activity of IFN-lambda and IFN-gamma on HBV in comparison to IFN-alpha in vivo in the humanized uPA/SCID mouse model to also get further insights in the antiviral activity of IFN-alpha. By comparing pegylated IFN-alpha with pegylated IFN-lambda, both interferons showed a similar strong induction of interferon-stimulated genes (ISGs) in human hepatocytes. However, a remarkable and persisting suppression on HBV with reduction of serological and intrahepatic parameters was only observed for treatment with PEG-IFN-alpha. These results suggest that the induction of ISGs does not seem to be sufficient to establish a strong and sustained antiviral effect on HBV, thus indicating that PEG-IFN-alpha uses a mechanism apart from innate signaling to induce HBV suppression. In contrast to PEG-IFN-lambda, treatment with PEG-IFN-alpha promoted re-detection of the host factor SMC5/6 complex in human hepatotocytes. SMC5/6 complex is assumed to act as a viral restriction factor repressing cccDNA transcription. However, in the infection process it is degraded by HBx to alleviate the SMC5/6-mediated repression. Detection of the complex suggests a reduction of the HBx protein by PEG-IFN-alpha, which seems to be of crucial importance for the antiviral activity of PEG-IFN-alpha as the reduction is not seen with PEG-IFN-lambda. Underlying mechanisms for the reduction need to be further investigated. First comparative studies on the short-term impact on IFN-gamma und IFN-alpha showed a similar antiviral activity for both interferons. For profound interpretations, however, observed effects need to be repeated with more mice as well as for a longer treatment period.
